Part 1: About PMS

Understanding PMS is one of the most important things you can do to control their symptoms. In this booklet, Jarret D. Morrow, MD, scientific director and president of the University of Industry Health, presents the facts about PMS, a summary of treatment options and ongoing independent research supports the use of SPM ™ to relieve Zenstral premenstrual syndrome and premenstrual dysphoric disorder.

What are PMS and PMDD?

Premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD), a more severe form of premenstrual syndrome, are common and potentially debilitating conditions that many women suffer. Hormonal changes after ovulation trigger a disturbing set of symptoms appear about 14 days before your period. PMS and PMDD affects an estimated 40 million women in the United States, with more than 5 million require medical treatment.

What causes PMS?

Although the exact cause of PMS is not completely understood, it appears that the hormones progesterone, estrogen and testosterone are involved, along with changes in the level of the brain chemical serotonin. Nutritional deficiencies in magnesium and vitamin B6, hormonal changes, neurotransmitter activity, diet and stress have been implicated in the etiology of PMS.1, 2

What are the symptoms of PMS?

Most women feel some discomfort before their periods. But if you have PMS may feel so anxious, depressed or uncomfortable that you can not cope at home or at work. Common symptoms of PMS include:

Symptoms usually stop or slow down quickly after your period starts, and may be free of symptoms until about two weeks before your next period.

What are the risks of PMD and PMDD?

PMS and PMDD are not mere inconveniences. These conditions may adversely affect personal success, professional and economic advancement of women. Women who suffer from PMS have an increased risk of depression, absenteeism, strained relations, reduced productivity, and worsening of medical conditions such as epilepsy, asthma, fibromyalgia, multiple sclerosis or migraine headache in the phases specific menstrual cycle.

How I can know if I have PMS?

According the American College of Obstetricians and Gynecologists (ACOG), can have PMS if

  • PMS symptoms occur only during the last two weeks of your cycle.
  • The symptoms are uncomfortable enough to impair their quality of life.
  • other disorders that may be confused with PMS have been excluded.

If you suspect you have PMS, ACOG recommends keeping a diary of your symptoms for two or three consecutive months to help in diagnosis. It is important talk to your doctor to confirm diagnosis and rule out other possible causes of your symptoms, including depression, migraine headaches, seizures, bowel syndrome irritable, chronic fatigue syndrome, or allergies.

What I can do to control my symptoms of PMS?

Women often can control symptoms of premenstrual syndrome by making changes in the way we eat, exercise, and focus their daily lives. To reduce discomfort, try applying as many of the suggestions below as possible.

Dietary advice to control the SPM

  • Eat small, frequent meals to reduce the swelling and the feeling of satiety.
  • Eat carbohydrates complex as whole grain breads, pastas and cereals.
  • Cut down on sugar and fat in your diet.
  • Avoid salt for the last few days before your period to reduce the swelling and fluid retention.
  • Avoid caffeine to feel less tense and irritable.
  • Avoid alcoholic beverages. The consumption of alcohol before period can make you feel more depressed.
  • Choose foods rich in calcium.
  • Increased consumption of dietary fiber.

Lifestyle Tips for Managing PMS

  • Get aerobic exercise. Regular daily exercise will help improve your overall health and can relieve symptoms such as fatigue and depressed mood.
  • Ensuring adequate eight hours of sleep per night.
  • Try to schedule stressful events for the week after your period.
  • Maintain a regular schedule for eating, sleeping and exercise.
  • Practice deep breathing exercises to help to reduce headaches, anxiety or trouble sleeping.
  • Avoid cigarette smoking.

What medical treatments are available for PMS and PMDD?

There are four types of prescription drugs used for PMS and PMDD:

  1. The antidepressants that act on serotonin, including fluoxetine, sertraline and paroxetine.The recently FDA ordered the manufacturers of antidepressants to issue warnings of black box that patients with these drugs should be closely monitored for "clinical worsening suicide, or unusual changes in behavior."
  2. Agents inhibiting ovulation, such as danazol, estradiol, transdermal patches, and some oral contraceptives. These drugs carry risks of serious side effects can be very expensive.
  3. Other psychotropic drugs to reduce anxiety, such as alprazolam benzodiazepines and other anxiolytics. From alprazolam has a short half-life life, you run the risk of addiction in patients who start this medicine.
  4. Spironolactone, a diuretic that is available by prescription only care. It has been proven effective in reducing premenstrual water retention, but does not address the other symptoms of premenstrual syndrome.

How effective are selective reuptake inhibitors (SSRI) for the treatment of premenstrual syndrome?

SSRIs are a class of drugs used to treat depression, anxiety and personality disorders. Patients may experience many side effects the use of these medications as weight changes, increased anxiety, suicidal thoughts, and sexual dysfunction. A study published in the Cochrane Database concluded convincing evidence exists to support the use of selective inhibitors of serotonin reuptake in the management of severe premenstrual syndrome. However, withdrawals due to side effects were 2.5 times more likely to occur in the treatment group than in the placebo group.3

What is the effectiveness the use of analogs of gonadotropin-releasing hormone (GnRHa) to treat PMS?

GnRH analogues to stop work periods to decrease estrogen production. A recent meta-analysis published in the British Journal of Obstetrics and Gynecology concluded that treatment with GnRHa seems to be an option effective management PMS.4 Unfortunately, patients who take these analogues may experience some serious side effects, including type of menopause symptoms such as hot flashes, mood swings, vaginal dryness and increased LDL cholesterol and bone loss.

It is the bright light therapy effective in treating depression associated with premenstrual dysphoric disorder?

Light therapy involves exposure to full spectrum light for a certain amount of time. A recent meta-analysis of bright light therapy published in the American Journal of Obstetrics and Gynecology concluded that "current evidence does not justify a broadcast enthusiastic and confident about the rejection of this therapeutic modality. "5 In essence, this suggests that more research is needed to study the potential benefit of this treatment for PMS.

What is a meta-analysis?

A meta-analysis involves collecting data from many studies of similar research, then analyze the data collected for significance statistics.

What tests are used in the formulation of Zenstral SPM ™?

Below is a representative sample of the scientific evidence used in the formulation of SPM ™ Zenstral:

Extract Vitex Agnus castus (VAC)

A recent double-blind, randomized controlled trial published in the British Medical Journal showed a 52 percent reduction in PMS symptoms in women treated with the dry matter of agnus castus fruit.11 Similar results were obtained in another study published in the Archives of Gynecology and Obstetrics.12

Another recent study Vitex agnus castus are extract as effective as fluoxetine in the treatment of PMDD, a more severe form of premenstrual syndrome. The authors reported that 57.9 percent of patients responded to VAC extract in the treatment of this disorder, and also found that VAC extract was more effective than fluoxetine in the treatment of physical symptoms PMDD.13

German health authorities have approved the use of Chasteberry for three gynecologic conditions: menstrual cycle irregularity, premenstrual syndrome (PMS) and breast pain (chest pain) .14

Magnesium

A recent study randomized, double-blind concluded that magnesium works synergistically with vitamin B6 to relieve symptoms of anxiety associated with PMS.15

Vitamin B6

The British Medical Journal did a meta-analysis of vitamin B6 in the treatment of premenstrual syndrome and concluded that it was likely to be beneficial in the treatment of premenstrual symptoms and premenstrual depression.16

Who should not take Zenstral SPM ™?

Consult a physician before use if you are pregnant, nursing, birth control pills, have a medical condition such as hormone-sensitive breast cancer, or are taking dopamine-related drugs.

How should I take?
